Bleeding while pregnant?

I am 39 weeks pregnant and i started spotting saturday night,i went to my doctor and all she told me was ok.yea don't like my doctor to much.but yesterday i started bleeding heavy to were i have to use a pad.i mean dark red i can actually feel when it comes out.than on top of that around 2 this morning i stated getting bad cramps to were i cant move or talk.I thought they might have been contractions but they just come out of the blue like first they were every 20 min now there just whenever.and i already lost my mucus plug so it's not that and i havn't got a exam since saturday.and these cramps when i get them are PAINFULL!!!!!what should i do?
